The Blind Eye is a campaign quest under Act 6: Dance of the Makers in Diablo 4.
Your objective is to get out of a nightmare realm imposed by Lilith.
Here’s a walkthrough of The Blind Eye in Diablo 4.
Use the Sightless Eye
You will have to use the Sightless Eye to track Lilith’s whereabouts.
During the cutscene, you’ll see Lilith talking to Astaroth and the latter granting her passage to her father’s domain (Mephisto’s domain).
She will then turn around as she spots the main character; trapping him into a nightmare realm.
Explore the nightmare
You will now need to explore the nightmare.
Walk all the way and you’ll see the priest from the beginning of the game. Speak to the priest.
Defeat the villagers of Nevesk
You will then need to defeat the villagers of Nevesk just like you did at the start of the game.
Continue exploring
After defeating the villagers, continue exploring the area.
You will see Vigo along the way. It’s just a memory from before.
Inspect Rathma’s corpse
Keep going and you’ll see Rathma’s corpse. Interact with it.
Keep going and you’ll see more familiar faces including Elias.
Inspect Lilith’s Altar
Proceed ahead and inspect Lilith’s Altar.
Defend against Lilith’s attack
As the main character refuses to join with Lilith, you have to defend against the demons.
They will keep spawning up to a certain point.
Enter the fiery portal
Mephisto interrupts the nightmare and opens a portal. Enter the fiery portal.
Inspect the campfire
You will find yourself at the beginning of the game where you first saw Mephisto in the wolf form.
Find the way out
Now that Mephisto has given you the ability to find the way out, continue following the path.
Defeat the nightmares
As you attempt to escape, you have to face a few bosses from before, but they’re not as tanky of course.
Defeat Genbar the Shrine Keeper.
Then, Elias will appear. Take him down as well.
You have to defeat Astaroth too. All of the bosses have low HP this time.
Use the Sightless Eye
You will finally reach the Sightless Eye. Use it to leave the nightmare.
Speak to Neyrelle after escaping.
The quest ends here. You’ll earn XP and coins.
