Epilogue: From The Wound Spilled is the last chapter in Diablo 4.
Your objective is to find Neyrelle as she took the soulstone containing Mephisto.

Promises
The first quest is called Promises.
Find Neyrelle at the hidden chapel. You’ll discover that she’s gone. Talk to Lorath instead.
Leave the chapel and you’ll see Iosef and the Knights Penitent outside as they want to arrest Lorath.
Slay them all instead.
A Heavy Burden
For the next quest A Heavy Burden, go to the Horadric Vault as marked in the map below.
Enter the Horadric Vault and inspect the Runed Letter left behind by Neyrelle.
Legacy of the Horadrim
The next quest is Legacy of the Horadrim. Go to Firebreak Manor to meet Lorath as marked in the map below.
Speak with Lorath.
During the cutscene, he’ll be able to decipher what Neyrelle had written and that she plans to take the soulstone far away.
The epilogue of Diablo 4 ends here. You’ll now unlock Whispers of the Dead which is a priority quest.
