Knee-Deep in Filth is a campaign quest under Act 5: Secrets Bartered, Fates Told in Diablo 4.
Your objective is to chase after the fleeing Elias.

Catch up to Taissa
In the previous quest, Elias runs away after losing the fight. Taissa goes after him.
You’ll then need to catch up to Taissa.
Keep going and you’ll see her waiting on the way.
Track Elias
Together with Taissa, you have to track Elias through the Plaguemarsh.
Follow the path and you’ll eventually see Elias who is badly wounded. A cutscene will load here.
The Tree of Whispers will claim his head.
Talk to Taissa
Speak with Taissa after the cutscene.
The quest ends here. You’ll earn XP and coins.
