Wrack and Ruin is a campaign quest under Act 5: Secrets Bartered, Fates Sold in Diablo 4.
Your objective is to find the town of Yngovani.

Find the town of Yngovani
First of all, head to the yellow circle area on the map to find Yngovani.
You should then find an Engraved Stone Door in the area.
Return the Right Eye
You will then need to find the Right and Left Eyes.
Head to the Right Eye first which is the smaller circle.
You will see an altar. Get the Right Eye from here.
Return the Left Eye
Now, head north to find the Right Eye. You should see a group of serpents near a survivor. Kill them.
Talk to the survivor and get the Left Eye.
Bring them to the gate
Go back to the gate with both of the eyes to open it.
The quest ends here.
