Dark Omens is a quest under Act 2: The Knife Twists Again in Diablo 4.
Your objective is to investigate the disturbance at Firebreak Manor.

Head to Firebreak Manor
Go up north from the Keep to Firebreak Manor. The location is marked in the map below.
Talk to Steward Wilfred who is at the main door.
Enter Donan’s Study
Head inside Donan’s study.
You will then see blood petals. Inspect Lilith’s Blood Petals.
You will then see a cutscene between Donan and Lilith.
Defeat goatmen attackers
Leave the study and you’ll see the manor being attacked by goatmen. Defeat them all.
Talk to Donan outside after the attack is over.
